Super Member Awards and Primary 4 Hearts Pins

This is a club level award presented to any member who completes the required number of actions and fills out the form. It’s a fun checklist to encourage youth to try everything 4-H has to offer and practice record keeping—all while learning, leading, and having lots of fun along the way!

There’s a separate form for Primary members (4-H ages 5-8) and one for Junior, Intermediate, and Senior members (4-H age of 9 and up). Your "4-H age" is your age on December 31st of that 4-H year.

Put the sheet in the front of your record book so we can make a note at Club Judging, and award the certificate. You will put your Super Member Award certificate in the Honors section of your record book.
