Super Member Challenge and Primary 4 Hearts Pins
This is a club level award presented to any member who completes the required number of actions and fills out the form. It’s a fun checklist to encourage youth to try everything 4-H has to offer and practice record keeping—all while learning, leading, and having lots of fun along the way!
There’s a separate form for Primary members (4-H ages 5-8) and one for Junior, Intermediate, and Senior members (4-H age of 9 and up). Your "4-H age" is your age on December 31st of that 4-H year.
Put the sheet in the front of your record book so we can stamp completed items at Club Meetings. You will put your Supermember Award certificate in the Honors section of your record book.
